Teaching Excellence Award

Format for Submissions
While the criteria for the Teaching Excellence Award are the most important ingredients measured by the various reviewing groups, the physical format of the submission is important insofar as it does not detract from the examination of the material. Therefore, submissions are requested to adhere to the following format.
1. Nomination packages should be contained in a plastic comb binding.

2. The first page should be a cover page, which includes the name of the Teaching Excellence Award, the year submitted and the nominee's name.

3. Page two should include the nomination form.

4. Page three should include a table of contents.

5. The next section should contain at least three but not more than four letters of support; preferably at least one should be written by a faculty member from within the nominee's division at his or her institution. Each letter is limited to 500 words and should be written with the consideration of the criteria of the award in mind.

6. The next section should include that material from the nominee that supports his/her nomination. This material should be limited to 10 pages. Material in this section may include (but is not limited to):

a. a personal statement on teaching excellence as it relates to the criteria of the award, and past and present practice. Evidence of currency in both discipline and pedagogy should be stressed;
b. a resume or vita;

c. a listing of awards and honors;

d.  evidence of innovation and leadership in teaching and service. This may include cooperative ventures with other institutions, innovation in classroom teaching with proven success, regional service activities, etc.

7. The final section should be devoted to student evaluations of faculty. This should include a minimum of four courses representing both upper and lower division covering the three most recently completed evaluation periods. Summaries of student evaluations should be used. Individual student evaluations should be available upon request.
Any additional materials to be submitted should be limited to five pages in length and provided in a separate binder.
